Congratulations on being invited to interview with Stambush! Here is the list of documents we must have in order to work together:  

    Resume
    Professional License
    CPR card - front and back
    Auto Insurance
    Original and Copy of Driver's License
    Original and Copy of Social Security card, or USA Passport (current), or USA Birth Certificate, or Green Card
    Check (for direct deposit)
    TB Test Results
    Immunization Records (MMR, Varicella, Tdap, Flu)
